Vipers Netball Club

Bluecards are required by all representatives of Vipers Netball Club who are 18 years of age or over. Representatives of Vipers who require a current blue card are:

• Managers

• Coaches

• Committee Members

• Umpires (over the age of 18)

Please note: Bluecards expire after two years and a new form needs to be completed.

To complete your Blue Card form (New/Renewal)

1. Download and complete the Bluecard Application OR complete the form online and print it out.
2. Provide copies of your Identification documents as listed on the Bluecard form.
3. Return the form to the Vipers Secretary who will organise submission of the form to the authorities.

For Teachers and Police Officers (Exemption card)

1. Please complete an Exemption Form OR complete the form online and print it out.
2. Provide copies of your Identification documents as listed on the Exemption form.
3. Return the form to the Vipers Secretary who will organise submission of the form to the authorities.

If you have a blue card for another organisation you will need to complete the following form to link your card to our organisation:

1. Please complete the Link Existing Card Form OR complete the form online and print it out.

2. Return the form to the Phoenix Registrar who will organise submission of the form to the authorities.
